BBC - Dorset - Nature. Jim Cronin's sudden death in March 2. But none more than his wife, Dr Alison Cronin. Alison is now in sole charge of Monkey World, the ape rescue centre near Wareham. She spoke to BBC Dorset about her late husband's legacy and how she plans to fight on for monkeys suffering abuse and neglect. Jim Cronin, founder of Monkey World. The early days. Monkey World might have started out small, but the reception the couple received in Dorset was vital for the centre's survival. The level of local support was proven time and again, and allowed Jim to expand the park as he rescued more and more monkeys. Protecting monkeys from abuse. Protecting primates from abuse and neglect was always Jim's driving force. That ranged from preventing abuses found in the pet industry in this country, to saving the victims of animal smugglers in South East Asia. A few years ago, Jim and Alison discovered the largest smuggling operation of orangutans in the world. They posed as buyers, going in to the markets and collecting evidence which they presented to the Thai authorities. Alison 3. Help playing audio/video. Most of the monkeys they have saved have been repatriated to countries like Indonesia. But some come back to live at Monkey World in Dorset. Alison's promise to Jim. Before Jim died, Alison promised to continue the work of Monkey World. She is looking forward to going to Chile this summer to bring back some capuchin monkeys. Alison 6. Help playing audio/video. Jim had been planning the trip before he died. Alison says that if this trip is successful, there will be another group of monkeys owing Jim their safety and protection. Jim's impact and legacy. Royal Marine Jez Hermer is one of those rallying around Alison to see her through this difficult time. He befriended the couple when he tried to rescue a baby chimp in Africa. Calling Monkey World on a satellite phone on a UN mission, he enlisted the help of Jim and Alison to nurture the chimp back to full health. Jez says that Jim's lasting legacy is that he changed people's attitudes towards conservation. Jez 1. Staying Power – I love the longevity of this kajal as it’s amazing and stays put for a really long time. It stays for 4 hours without concealer on my waterline and for 5- 6 hours with concealer/compact powder. On my upper lid, it stays for 6- 7 hours easily. The Colossal Super Black lacks in staying power on my waterline. Texture – I have no complaints with this kajal when it comes to the texture as it’s creamy and glides on smoothly on my eyes. Although, if you do twist it up a lot and apply with pressure, the tip is going to break (my heart broke along with it!). The texture of this kajal is pretty similar to Maybelline Colossal Super Black Kajal and Lakm. The 3 kajals are the same when it comes to pigment, too. The only difference between the Lakm. I am here again to review another product from Himalaya Herbals. It has been many years since I began having acne and in these many years, I have tried so many products to get rid of acne that I have forgotten the names of them. Himalaya Herbals Neem Face Wash was the first face wash I ever used. My fight- against- acne began with this face wash and I kind of got emotional seeing the transparent green tube of this neem cleanser again. I felt like saying, “Good to see you again, old friend.”Himalaya claims: A soap- free, herbal formulation that gently removes impurities and prevents pimples. Neem, well- known for its anti bacterial properties, kills problem causing bacteria, and Turmeric effectively controls acne and pimples. Use regularly for clear, soft and pimple free skin. How to Use Himalaya Neem Face Wash: Moisten face and neck; apply a small amount of Purifying Neem and gently work up a lather using a circular motion. Wash off and pat dry. Use twice daily. Use Himalaya Purifying Neem Pack once a week to rejuvenate and revitalize your skin. My Review: Purifying Neem Face Wash comes in a transparent plastic tube which has a dark green colored flip- open cap. The packaging is very simple and neat. The face wash is light green in color and has the appropriate consistency. The face wash lathers well and a small quantity is enough to cover the face and neck. Himalaya Neem was my first face wash and I used to like it a lot, initially. But later, I replaced this face wash with Clean & Clear Foaming Face Wash. At present, I have again used this neem face wash for 1. I do not know if Himalaya Neem actually has turmeric but I am certain that there are neem extracts in this. The face wash cleanses the face properly and leaves a faint neem smell for a while. It generously removes dirt, grease and oil from the face. It does not clog the facial pores. It works to slow down the occurrence of pimples but it has near to no effect on the existing pimples. However, the face wash cannot battle against oil. Right after using the face wash, my face begins getting oily again within 2. This thing bothers me a lot. Still, considering its price, I cannot complaint much. Himalaya Purifying Neem is budget friendly and an average face wash. Pros of Himalaya Herbals Purifying Neem Face Wash: Does almost everything as claimed. Has a mild smell. Suits all skin types. Has neem extracts. Cleanses the face nicely. Does not cause break outs. Helps to slow down the rate of occurrence of pimples, to some extent. Herbal and natural. Lathers well. Cons of Himalaya Herbals Purifying Neem Face Wash: Has no effect on existing pimples. Does nothing for acne/pimple marks. Does not control oil for more than 1. Not completely reliable to get rid of acne or pimples. Does not actually give clear, soft and pimple free skin. Price: Rs. 5. 5 for 5. VNA Rating: 2. 5/5. Conclusion: I am attached to Himalaya Neem Face Wash a bit emotionally. But now, I remember why I stopped using this neem face wash. When Himalaya Herbals improved the packaging few years ago, I felt that this face wash might also have been improved! It is all the same. I have not used this face wash along with Himalaya Purifying Neem Pack as directed. Despite its failure as a . Acne- prone skin people sometimes get weary using all the harsh or medicated acne/pimple products and also cleansers. They can use it occasionally as a harmless, refreshing and mild face wash. Also this can be used by girls with normal and dry skin. It is so versatile, in a way! Have you tried Himalaya Purifying Neem Face Wash? Share your reviews with is. ![]() NCLEX 3. 50. 0: Cardiac . Left bundle- branch block, aortic stenosis, and patent ductus arteriosus cause a paradoxical S2 split. Heard only on expiration, a paradoxical S2 split results from delayed aortic valve closure. A client with an acute myocardial infarction is receiving nitroglycerin (Tridil) by continuous I. V. Which statement by the client indicates that this drug is producing its therapeutic effect? Headache is an adverse effect of nitroglycerin. The drug shouldn’t cause a tingling sensation around the mouth and should lower, not raise, blood pressure. A client with refractory angina is scheduled for a percutaneous transluminal coronary angioplasty (PTCA). The cardiologist orders an infusion of abciximab (Reo. Pro). Before beginning the infusion, the nurse should ensure the client has: 1. Before abciximab is administered, the client should have an up- to- date PTT result available. The drug isn’t contraindicated in clients with a seizure history. Abciximab isn’t an opioid; therefore, an opioid antagonist doesn’t need to be at the bedside. Any client with refractory angina should be on continuous ECG monitoring; however, monitoring isn’t a requirement for administering abciximab. Allow the client uninterrupted periods of rest as much as possible. Limit the client’s visitors to coincide with CCU visiting policy. Move the client to a room far from the nurses’ station to reduce his exposure to noise. Correct Answer: 1 RATIONALES: Assigning the same nurse to the client when possible provides continuity of care and stability, thereby reducing his anxiety. The client needs uninterrupted periods of rest; however, providing as much rest as possible may leave the client feeling isolated. Feelings of isolation can increase the client’s anxiety, and having visitors can help distract the client. A room close to nurses’ station would provide this client with a sense of security because the nurses are close by in the event of an emergency. Before administering digoxin (Lanoxin), a nurse reviews information about the drug. ![]() She learns that after digoxin is metabolized, the body eliminates remaining digoxin as unchanged drug by way of the: 1. Therefore, a client with renal dysfunction will require a decreased digoxin dosage. Although some drugs may be eliminated by other routes, digoxin isn’t known to be eliminated by way of the lungs, feces, or skin. The nurse determines that a hockey player hospitalized with bilateral leg fractures is hemodynamically stable and observes the following pattern on the electrocardiogram (ECG) monitor. Which nursing intervention is most appropriate at this time? None; this arrhythmia is benign 2. Administering atropine sulfate, 0. Continuing to monitor for lengthening PR intervals 4. Evaluating the client’s recent serum electrolyte studies. Correct Answer: 1 RATIONALES: This ECG shows sinus arrhythmia with a rate of 7. In this benign arrhythmia, the rhythm is irregular; the impulse originates in the sinoatrial node and travels down the conduction system normally. The P- P interval is irregular; a P wave precedes every QRS complex; and the R- R interval is irregular, increasing with inspiration and decreasing with expiration. Sinus arrhythmia commonly is associated with vagal inhibition caused by respiration. It seldom causes symptoms and doesn’t call for atropine or other treatment. Continuing to monitor for lengthening PR intervals isn’t necessary because the PR interval doesn’t increase with sinus arrhythmia. Because this arrhythmia isn’t caused by an electrolyte imbalance, evaluating serum electrolyte studies isn’t warranted. The nurse would advise the client with an axillofemoral bypass graft to avoid: 1. Prolonged sitting can kink the femoral portion of the graft. Prolonged standing and upper- extremity exercises need not be avoided. The nurse administers basic cardiac life support to a client in cardiac arrest. Which action does the nurse perform? Assessing the patency of the airway 2. Administering I. V. Administering a countershock of 2. Breathing for the client after inserting an endotracheal (ET) tube. Correct Answer: 1 RATIONALES: A nurse certified in basic cardiac life support can assess airway patency. Administering a countershock of 2. ET tube are measures carried out during advanced life support. The nurse is caring for a cardiac client who requires various cardiac medications. When the nurse helps the client out of bed for breakfast, the client becomes dizzy and asks to lie down. The nurse helps the client lie down, puts up the side rails, and obtains the client’s blood pressure, which is 8. Hg. It’s time for the nurse to administer the client’s medications: nitroglycerin, metoprolol (Lopressor), and furosemide (Lasix). Which action is best taken by the nurse? Withhold the medications and notify the physician. Administer the medications immediately. Encourage the client to sit up and eat breakfast. Administer the nitroglycerin and metoprolol and withhold the furosemide. Correct Answer: 1 RATIONALES: The nurse should withhold the three medications and notify the physician. Each of these medications has the potential to lower the client’s blood pressure. Administering them together when the client is already hypotensive may severely lower the client’s blood pressure. The client may continue to experience dizziness when sitting up so breakfast should be held until his blood pressure stabilizes. A client with a permanent pacemaker and a long history of cardiac disease is admitted to the coronary care unit for evaluation for heart failure. The nurse observes the following electrocardiogram (ECG) pattern. What does this pattern indicate? Use of a DDD pacemaker with a rate of 7. Use of a VVI pacemaker with a rate of 7. Use of an AVI pacemaker with a rate of 7. Use of an AAI pacemaker with a rate of 8. Correct Answer: 2 RATIONALES: In the pacemaker identification code, the first letter stands for the heart chamber being paced (atrium, ventricle, or both . This ECG indicates use of a VVI pacemaker, which paces and senses the ventricle and is inhibited by a sensed event (a spontaneous QRS complex). A spike precedes every QRS complex stimulated by the pacemaker. Sensing that the client’s intrinsic rate is below 7. The other options give incorrect rates; also, if the atrium were being paced, a spike would precede each P wave, indicating atrial contraction. During digoxin (Lanoxin) therapy, the nurse should closely monitor the client’s: 1. This is because hypokalemia or hypomagnesemia can predispose the client to digitalis toxicity. Glucose and ketones aren’t usually in the urine except in a client with uncontrolled diabetes, and digoxin isn’t known to affect these levels in the diabetic. CK levels may be elevated if digoxin is administered I. M., but this route of administration isn’t recommended. A client is participating in a cardiac research study in which his physician is directly involved. Which statement by the client indicates a lack of understanding about his rights as a research study participant? The client has the right to withdraw from a study at any time without penalty. All information provided by the client will be kept confidential and used only by members of the study team for scientific purposes. The client must be informed of all risks associated with study participation. A client with chest pain receives nitroglycerin on the way to the acute care facility. Based on an electrocardiogram obtained on admission, the physician suspects a myocardial infarction (MI) and prescribes I. V. A primary goal of nursing care for this client is to recognize life- threatening complications of an MI. The major cause of death after an MI is: 1. Heart failure, in contrast, accounts for 3. MI deaths. Pulmonary embolism, another potential complication of an MI, is less common. The home care nurse visits a client diagnosed with atrial fibrillation who is prescribed warfarin (Coumadin). The nurse teaches the client about warfarin therapy. Which statement by the client indicates the need for further teaching? The client should limit his intake to one to two servings per day. Last week, my staff nutritionist Laura Schoenfeld wrote a guest post for my blog called “Is a Low- Carb Diet Ruining Your Health”. Perhaps not surprisingly, it has caused quite a stir. For reasons I don’t fully understand, some people identify so strongly with how many carbohydrates they eat that they take offense when a suggestion is made that low- carb diets may not be appropriate for everyone, in all circumstances. In these circles low- carb diets have become dogma (i. Followers of this strange religious sect insist that everyone should be on low- carb or even ketogenic diets; that all carbohydrates, regardless of their source, are “toxic”; that most traditional hunter- gatherer (e. Paleolithic) societies followed a low- carb diet; and, similarly, that nutritional ketosis—which is only achievable with a very high- fat, low- carb, and low- protein diet—is our default and optimal physiological state. Cut through the confusion and hype and learn what research can tell us about low- carb diets. On the other hand, I’ve also observed somewhat of a backlash against low- carb diets occurring in the blogosphere of late. While I agree with many of the potential issues that have been raised about low- carb diets, and think it’s important to discuss them, I also feel it’s important not to lose sight of the fact that low- carb diets can be very effective therapeutic tools for certain conditions and in certain situations. With this in mind, here are 7 things I think everyone should know about low- carb diets.#1: Paleo does not equal low- carb, and very low- carb/ketogenic diets are not our “default” nutritional state, as some have claimed. Some low- carb advocates have claimed that most traditional hunter- gatherer societies consumed diets that were very low in carbohydrates. ![]() ![]() ![]() I’ve even seem some suggestions that nutritional ketosis was “the norm” for these cultures. These claims are false. The majority of studies have shown that traditional hunter- gatherer (HG) societies typically consume between 3. Yet even these cultures—such as the traditional Inuit—often made an effort to obtain carbohydrates from berries, corms, nuts, seaweed, and tubers whenever they could, as Richard Nikoley has recently detailed on his blog. What’s more, contrary to popular claims, studies have shown that it’s unlikely the Inuit spent much time—if any—in nutritional ketosis. Their high protein intake would have prevented ketosis from occurring. With virtually no historical examples of human beings following ketogenic diets for any significant length of time, and few examples of very low- carb diets, it’s difficult to imagine how these diets could be considered our “default” nutritional state or the optimal approach for most people.#2: Low- carb diets are incredibly effective in certain situations. Lest low- carb advocates think that I am anti- low- carb, I’d like to reiterate that both the research and my clinical experience suggest that low- carb diets can be incredibly effective therapeutic tools for certain conditions. These conditions include (but aren’t limited to): Overweight and obesity. High blood sugar, metabolic syndrome, diabetes (both type 1 & type 2)Traumatic brain injury. Epilepsy. Parkinson’s disease. Alzheimer’s disease. Other neurological conditions. PCOSI have personally witnessed some remarkable transformations using ketogenic diets therapeutically in my practice. I recall an 8. 4 year- old woman who came to see me complaining of dementia and early- onset Alzheimer’s. She was losing her memory and cognitive abilities at an alarming rate. After just two weeks on a ketogenic diet, this progression not only halted, it reversed: her memory returned, her mind was sharper, and she was far less confused and disoriented. Her family (and her doctor) were stunned, and could hardly believe the changes they were seeing. Yet as impressive as very low- carb (VLC) and ketogenic diets can be in certain situations, that does not mean that these diets may not have some undesirable side effects over the long term—some of which we’re only beginning to understand. For example, as I discussed with Jeff Leach from the American Gut project in a recent podcast, some preliminary research suggests that long- term ketogenic/VLC diets may cause adverse changes to the gut microbiota. It’s worth noting that many of these fibers are found in foods with moderate to high carbohydrate content—foods that would typically be excluded on very low- carb diets. It’s important to note, however, that the beneficial bacteria- starving effects of ketogenic/VLC diets can be at least partially offset by consuming non- digestible, fermentable fibers like resistant starch and non- starch polysaccharides that don’t count toward daily carbohydrate intake. ![]() ![]() The 100 by Jorge Cruise (2013): What to eat and foods to avoid. To quote Robert Atkins on the topic of refraining from alcohol while on a diet: “Here’s the problem with all alcoholic beverages, and the reason I recommend. If you enjoy a cocktail before dinner, or a glass of whiskey by the fire, you'll be happy to know that most alcohols are zero carb! It is not calories but carbs that are the single most contributor to excessive weight. It is true that the body burns alcohol first, and therefore it is reasonable to. This is something I recommend to all of my patients following low- carb diets)#3: The fact that ketogenic/VLC diets work therapeutically for certain conditions does not make them appropriate in all circumstances, for all people. This assumption is a basic failure of logic, but it’s remarkable to see how often it happens. A person has a life- changing experience with a VLC diet, so they assume that their friend will have a similar experience. Or a clinician that works primarily with people suffering from neurological conditions has great success with ketogenic diets, and then makes the assumption that all people (regardless of their health complaints) will benefit from them. ![]() This is akin to saying that since people with hemochromatosis (a genetic condition that causes iron overload) need to limit their iron intake, everyone should consume foods that are low in iron. The belief that “everyone” will benefit from one particular dietary approach—no matter what it is—ignores the important differences that determine what is optimal for each person. These include variations in genes, gene expression, the microbiome, health status, activity levels, geography (e. When it comes to diet, there is no one- size- fits- all approach. Some people may thrive on a long- term, low- carb diet. I have patients and even a family member in this category. And maybe you’re one of them too. But that doesn’t mean everyone will have this experience. If you talk to practicing clinicians who work with patients on a daily basis, or spend any amount of time in internet forums or the comments sections of nutrition blogs, you’ll find numerous reports from people who either experienced no benefit from or were even harmed by following a low- carb diet. What blows my mind is that the “low- carb zealots” seem completely incapable of accepting these reports at face value. Instead, they’ll argue that anyone who doesn’t succeed with low- carb is either doing it wrong, cheating, or somehow imagining their symptoms. What’s the more likely explanation here? That everyone who gets worse with a low- carb diet is either incapable of following directions, weak- willed, or delusional? Or that a low- carb diet simply does not work for everyone? You be the judge. ![]() Pure fat and vegetable oils are free of all carbs. This includes butter, margarine, and all types of. Specifically, What Is The Ketogenic Diet? There are variations, but in a typical keto diet, 5% of daily calorie intake is derived from carbohydrates, 20% from protein. Robb Wolf’s 30 Day Paleo Transformation. Have you heard about the Paleo diet and were curious about how to get started? Or maybe you’ve been trying Paleo for a. Here’s an example: “A low- carb diet is effective for treating type 2 diabetes. Therefore, eating too many carbohydrates led to this condition in the first place.”This is like saying: Restricting iron is helpful in hemochromatosis patients. Therefore, consuming too much iron is what caused hemochromatosis in the first place. A low- FODMAP diet helps patients with Irritable Bowel Syndrome (IBS). Therefore, eating FODMAPs caused IBS in the first place. A low- histamine diet alleviates the symptoms of histamine intolerance. Therefore, histamine intolerance is caused by eating too many histamine containing foods. Or, more ridiculously, since wearing a cast on your arm will help the broken bone heal, the reason you broke your arm in the first place is because you weren’t wearing a cast. It’s true that VLC/ketogenic diets are effective for improving the metabolic markers associated with type 2 diabetes. But that doesn’t mean that eating too many carbohydrates led to the condition in the first place. It is certainly possible (and indeed likely) that eating too many refined and processed carbohydrates, in the form of flour and sugar, contributes to diabetes. But I have not seen a single study suggesting that eating whole- food carbohydrates (e. ![]() On the contrary, reviews of prospective studies looking at the relationship between fruit intake and diabetes have found that those with the highest intake of fruit had the lowest incidence of diabetes. For example, there are numerous studies showing that low- carb and ketogenic diets can help with weight loss and metabolic problems. Studies have also shown that calorie- restricted diets, protein- sparing modified fasts, and even low- fat diets can also be effective treatments. They may be able to reverse their condition by following a high- protein, moderate- carbohydrate, moderate- fat diet (such as the Paleo diet with 3. I just mentioned. In #1 above, I referenced studies indicating that most hunter- gatherer societies consumed about 3. These carbohydrates came from starchy tubers and plants, whole fruit, and in some cases, honey. We also have evidence of specific ancestral populations—such as the Kitava, traditional Okinawans, and Tukisenta—that consumed between 7. When it comes to macronutrients, quality is much more important than quantity for most people. ![]() Final thoughts. I hope this helps to clarify some of the confusion that has surrounded this issue. Low- carb diets are an effective therapeutic tool in certain situations, and one that I (and many other clinicians) use in my clinical practice. That said, it’s equally true that low- carb—and especially VLC and ketogenic—diets are not appropriate in all circumstances, and they are certainly not our “default” or optimal nutritional state. Sadly, it doesn’t seem to matter how much scientific evidence, clinical experience, and common sense is brought to bear on this question: those who preach and follow low- carb dogma will not be swayed. ![]() Lose up to 10lb in just two weeks: Get beach ready with our summer diet IN THE second part of our bikini detox programme, here's a simple two week meal plan that won.
